Terms and Conditions:

The Terms & Conditions of the Synergy Health Card Program, the medical practitioners/ service providers empanelled with Synergy Health Card (hereinafter referred to as 'SYNERGY HEALTHCARE') & the offers made on the Synergy Health Card Program may periodically change. We hence request you to check for these changes on the website & by calling us on our help line numbers on a regular basis. A Synergy Healthcare Member (hereinafter referred to as 'Member') is defined as an individual who has a valid and current Synergy Health Card issued by Synergy Healthcare.

Usage of the Synergy Healthcare Program by the Member implies that the Member agrees to the following terms and conditions of Service, Refund, Cancellation & other terms as incorporated herein and understands that these terms govern the use of Synergy Healthcare Program. All Members are understood to have read (on their own behalf and on behalf of their dependents/family/other members) the following terms and conditions and agree to abide by these Terms of Use. Synergy Healthcare reserves the right to modify, alter or otherwise update these Terms at any time and all Members agree to be bound by such modifications and/ or updates.

The Synergy Health Card provides the Member access to a network of voluntarily participating medical professional / dentists/pharmacies/ hospitals & other medical practitioners / vendors (hereinafter referred to as 'Network Members').

MEMBERSHIP TERM:
Once the initial "Membership Fee" and any enrolment, processing or other fee is paid and received, a Member will be entitled to the subsidised services during the membership period as defined by the expiry date given on the face of the Synergy Health Card (from the date of activation) or as otherwise stated on the valid / active Synergy Health Card.

RENEWAL OF MEMBERSHIP:
Members may be informed at least one month in advance before their card expiry date. It is the responsibility of the member to visit Synergy Healthcare Centre for membership renewal. SYNERGY HEALTHCARE is not responsible and assumes no liability for any disruption of service in case the payment gets delayed or the payment cheques are dishonoured. Card services will remain suspended till the realization of the payment.

RENEWAL CHARGES:
The charges for renewal can be confirmed by mailing out support team at [email protected] a month in advance of the expiry date. Kindly note that any Medical Services being offered at subsidised rates will not be offered at these rates by the Network Member as soon as the card validity has lapsed.

CANCELLATION:
Synergy Healthcare services are Non-Transferable and are not Refundable once user have pait for the Membership Plan. In all eventualities, decision of Synergy Healthcare will be final and binding on all the parties.

LOST MEMBERSHIP DETAILS:
In case of loss of Membership Details, a Duplicate Card may be issued at the centre with nominal charges. The tenure of the membership will remain as specified in the initial membership card.

COMPLAINTS:
Members are advised to address their complaints, if any, to our representatives. They are there to help you out. However if you are not satisfied, you may write to us at our corporate office address or mail us at [email protected]

The Member's rights or duties under this Agreement may not be assigned or delegated without the prior express written consent of Synergy Healthcare. Member agrees that he / she will use his / her Plan or Program Membership only for his / her personal benefit or for the benefit of his / her Family Members/others who have individual Synergy Health Cards for themselves (in case of Family or Couple plan). The Member is responsible for paying Network members for all services rendered unless otherwise agreed upon by the Member and Network Member.

SYNERGY HEALTHCARE offers different variants of Health Plans to customers and features/ benefits offered could vary from one variant to the other. Please visit our centre and refer to our detailed SYNERGY HEALTHCARE Plans to check if some benefits are offered on your SYNERGY HEALTHCARE Plans variant. In the case of a Couple or Family SYNERGY HEALTHCARE Plan membership, there needs to be one person designated at the time of applying as the Primary Member.

Synergy Healthcare & the Network Member who are part of the Synergy Healthcare Plan network reserve the right to revise their charges at any time.

Each Member, for himself / herself, and on behalf of any Family Member who uses the services under the Health Card hereby forever releases, acquits and discharges each of Synergy Healthcare and its employees, officers, directors, agents and affiliates from any and all liabilities, claims, demands, actions and causes of action that such Member or his / her legal representative(s) may have by reason of any damage or personal injury sustained as a result of or during the course of the use of any Medical Services.

Synergy Healthcare Plans offered by Synergy Healthcare are not health insurance policies. The Card provides subsidised services through its empanelled Network Members at our health care centre.The benifits like cashless insurance services are not available at Synergy Healthcare center at present, the user will be provided a detailed bill which the user can use to claim insurance services with their respective insurance companies or vendors. The Member is obligated to pay for all health care services from those Network Members who have contracted with Synergy Healthcare for providing services under the empanelment agreement.
